* src/server_session.c (format_exit_signal): Removed "Process
killed by" from the message.
(spawn_process): Rewrote function, using USER_SPAWN and struct
spawn_info. Updated the shell_request, exec_request and
subsystem_request handlers.
(init_spawn_info): New function.
(do_spawn_subsystem): Fixed argument list, should be -c <program>.
* src/unix_user.c (exec_shell): Added the real login shell to the
lsh-execuv argument list.

Reorganized server pty handling. Now it works also on Solaris.
Improved key reexchange handling. Should now follow the spec,
and request key reexchange once per hour (reexchange requests
after 1GB data not yet implemented).

Use the aes256-cbc cipher by default. Includes sparc assembler
code, and the C implementation has been optimized as well.
In interactive mode, the client modifyes the terminal's VMIN

disconnects if handshake and userauthentication is not
completed in about ten minutes.
Fixed the default port handling on systems that don't include
ssh in /etc/services.

Rewrote the code for executing user processes. Now it avoids
the sequence fork(), setuid(), exec(), which may leave server
memory readable by users between setuid and exec. Instead, the
server exec:s a helper program lsh-execuv that changes the uid
before exec:ing the shell.
